Physical Medicine and Rehabilitation (PM&R)
PM&R services include —

Occupational therapy

Physical therapy

Therapeutic recreation

Specialized clinics include amputee clinic, wheel chair clinic, diabetic shoe clinic, MOVE exercise program
Nursing Home Care Unit Admission Criteria
-
Any veteran who has a service-connected disability rated at 70 percent or more qualifies for nursing home care.
-
Veterans whose service connected disability is clinically determined to require nursing home care.
-
Care will be provided in a VA nursing home or contract nursing home.
Short Stay Rehabilitation (Purchased Skilled Care)
-
Time-limited, goal-directed care for the purpose of returning the veteran to functioning as independently as possible.

Services are rendered and goals achieved by interdisciplinary effort to improve function.

The primary resources for care are physical therapy, occupational therapy, kinesiotherapy, and speech/language pathology.
Homemaker Home Health Aide

The Homemaker Home Health Aide Program is a community based program that provides personal individualized care in the home. Services may include assistance with activities of daily living, aid in ambulation or transfer, active/passive range of motion, assistance with medical equipment and routine health monitoring.
Community Nursing Home
Contract Adult Day Health Care

Adult Day Health Care Program is a therapeutic day care program, which provides medical and rehabilitation services to disabled veterans in a congregate setting.

Adult Day Health Care is part of the basic benefits package. Veterans must be enrolled in the VA system for their health care and meet the clinical admission criteria.
Respite
Hospice & Palliative Care Program
-
Hospice/palliative care programs offer pain management, symptom control, and other medical services to terminally ill veterans or veterans in the late stages of the chronic disease process, as well as bereavement counseling and respite care to their families.
Home IV Infusion
